Service Agreement

Welcome to Roots & Refuge Center. This Service Agreement outlines the terms and conditions under which we provide therapeutic services. By engaging in our services, you agree to the following:

1. Nature of Services

Roots & Refuge Center provides mental health and wellness services, including counseling, workshops, and community care. Our services are not a substitute for emergency medical treatment or psychiatric intervention.

2. Confidentiality

All sessions are confidential. We do not share your information without your written consent, except as required by law (e.g., risk of harm, child abuse, or court order).

3. Appointments & Cancellations

Sessions are by appointment only.

We ask that you provide at least 24 hours’ notice for cancellations.

Missed appointments or late cancellations may incur a fee.

4. Fees & Payment

Fees will be communicated before the first session.

Payment is due at the time of service unless otherwise agreed.

We accept payments via PayPal or other approved methods.

5. Client Responsibility
Clients are expected to:

Attend sessions on time

Engage actively in the process

Communicate openly and honestly

6. Termination of Services

You may choose to end services at any time. We may also suggest termination if the service is no longer beneficial or appropriate.
